Example of Driving Licence application form D2
Example of Medical Examination Report form D4
In order to obtain an LGV or PCV provisional licence an application needs to be sent, along with a completed medical form, to the DVLA at Swansea. By far the best, easiest and almost certainly cheapest option is to come along to our offices and let us carry out the medical. Our company Doctor visits our premises on a weekly basis, generally during the weekend to conduct the medical.
When candidates arrive at our premises, at the agreed time, we help them with the completion of all the relevant forms to ensure that these are filled in correctly. If any mistakes are made when completing the forms, this will cause delays in the return of the provisional entitlement.
When attending for the medical, candidates will be supplied with a theory book (theory disc can be obtained at an additional cost) in order to start studying whilst waiting for the licence to arrive.
The theory test is now in two parts; a multi-choice test and a hazard perception test. In order to ensure that the hazard perception element of the test is understood, we have a classroom presentation to show to all candidates before the test is taken. This means that candidates clearly understand the system and our classroom facilities allow candidates to take as many mock theory tests as required.
Once the provisional licence is received, a theory test can be booked. We do have theory tests pre booked, so an appointment can be allocated at a suitable time and place.
When the theory test is passed, a driving test can be arranged and the relevant driver training can be organised.
